Headlight is in-network with several major carriers in Arizona and Oregon, including Aetna, Blue Cross Blue Shield of Arizona, Cigna/Evernorth, and UnitedHealthcare/Optum. In Oregon we also accept Regence Blue Cross Blue Shield, Moda Health, TRICARE/TriWest, and OHP Open Card (fee-for-service) only. We are not able to accept Arizona Medicaid.
Coverage varies by plan — even within the same carrier — so the most reliable way to confirm is to send your insurer and plan name through the intake form. We'll verify before your first visit.
Self-pay is available, and superbills are provided for out-of-network reimbursement.
